What the team does
- Work with stroke patient, their family, carers and the hospital stroke team to ensure an early planned discharge from hospital.
- Provide specialist stroke rehabilitation seven days a week in home and community environments.
- Provide specialist advice and education on stroke recovery and health promotion to patients, carers and families.
- Work with the stroke patient in preferred activities of daily living in order to help them reach their full rehab potential.
- The team will enable the patient to understand and learn to manage their long term condition.
How will it benefit me?
- The supported stroke discharge team aims to:
- Provide a smooth process between your rehabilitation on the ward and rehabilitation at home.
- Improve your ability to carry out activities important to you.
- Help you learn more about your condition.
- Improve your quality of life and support your family/carers/partner.
What is required of me?
- You will be expected to participate in your rehabilitation and practice tasks set by your therapists.
- Alongside the therapy you will be set therapy goals and when these goals have been addressed the team will then give you some advice on self-management.
- There is no set time as to how long this will take but will be dependent on your needs.
Rehabilitation and goals
- Alongside the therapists, you will set therapy goals in order to fully participate in rehabilitation and practice tasks.
- The team will work closely with you so that you understand and practice what you need to do to reach your rehab potential.
Planning for discharge from the team
- The team will work with other services to ensure support and rehab continues where needed.
- The team will work with the patient and carers to ensure they have the resources to be independent in managing their condition and long term goals.
- A letter summarising progress will be sent to the patient and the patient’s GP and consultant on discharge from the team.
What happens when I finish with the team?
- By the end of your rehabilitation with the team you will have a greater understanding of how to manage your condition. The team will have gone through with you how to do this and what areas of rehabilitation you can continue with.
- There are local support networks available for people who have suffered a stroke, details of which are in your information pack.
