We can provide rehabilitation and support if you’ve had a stroke. 

Our team are here to help make sure your transition from hospital back to your own home goes as smoothly as possible. We’ll work with your doctors, nurses, specialists, family and carers to make sure it’s safe for you to go home and that your rehabilitation can continue once you’re there.

Our teams includes:

  • occupational therapists
  • physiotherapists
  • speech and language therapists
  • neuropsychologists
  • rehabilitation assistants

We also have close links with social services and 3rd sector organisations such as the Stroke Association

When you return home, our team will contact you you within 24 hours, Monday to Friday, to arrange an assessment. During the assessment we’ll discuss your rehabilitation goals and create a treatment plan to help you regain your independence as quickly as possible.

This may include things like:

  • individual or group therapy
  • specialist advice and education to help your recover
  • advice on coping strategies for managing long term conditions
  • adapting your home or working environment
  • practicing and regaining old skills and activities
  • learning to do activities differently
  • tailored exercise and physical activities

During your rehabilitation we’ll regularly review your progress and adjust your support and treatment as your needs change.
